چکیده

A 38-year-old man was admitted for possible embolization of a left carotid cavernous sinus fistula. Two months previously, the patient had been admitted to the hospital after a motorcycle accident. Twenty-four hours later, he was noted to have left proptosis, chemosis, and a third-nerve palsy. An angiogram was performed and showed a carotid cavernous sinus fistula (Fig. 1 A). The patient was referred to our institution for embolization. Physical examination revealed marked left proptosis and chemosis, but no orbital bruit was present. A repeat angiogram failed to show the fistula (Fig. 1 B). Thrombosis of the left cavernous sinus was suspected, and two sets of MR images were obtained, one set before and one set after gadolinium-DTPA injection (Figs. 1 C and Fig. D).

Magnetic resonance imaging of the cavernous sinus.

The magnetic resonance (MR) appearance of the cavernous sinus (CS) was studied in 10 normal and 23 abnormal CSs(11 vascular and 12 neoplastic lesions) using T1-weighted spin echo images with and without Gd-DTPA. In normal CSs, the intracavernous carotid artery (ICA) was disclosed as an area of signal void that was not enhanced with Gd-DTPA. Most venous flow showed low intensity and was markedly...

Indirect carotid cavernous fistula presenting as pulsatile tinnitus.

This paper reports a case of spontaneous indirect carotid cavernous fistula that presented with pulsatile tinnitus, left-sided temporal headache and left-sided ptosis. The pulsatile tinnitus, its aetiology and investigation are discussed. The importance of pulsatile tinnitus is highlighted, with a discussion of carotid-cavernous fistulas. This case illustrates that clinically silent cavernous s...

Gadolinium-DTPA and MR imaging of pituitary adenoma: a preliminary report.

Gadolinium-DTPA MR imaging (Gd-MR), unenhanced MR imaging, and contrast-enhanced CT studies were compared prospectively in six patients with surgically confirmed pituitary adenomas and three patients without sellar pathology to determine the utility of Gd-MR in the diagnosis of pituitary adenoma. In normal patients, the pituitary gland, cavernous sinus, and infundibulum enhanced by T1 shortenin...

Spontaneous thrombosis and complete disappearance of traumatic carotid-cavernous fistulas after angiography.

Traumatic carotid-cavernous fistula (TCCF) is a direct shunting fistula due to a tear in the cavernous portion of the internal carotid artery (ICA). Spontaneous thrombosis of the high-flow shunts in TCCFs is extremely rare. Most cases are treated using endovascular embolization to relieve the clinical presentations. We report 2 unusual cases of TCCF with spontaneous closure of fistulas at inter...

Spontaneous carotid cavernous fistula presenting only with cranial nerve palsies.

PURPOSE To discuss the differences in angiographic findings between cases of spontaneous carotid cavernous fistula with and without the classical triad of symptoms (pulsating exophthalmos, bruit, and conjunctival chemosis). METHODS With CT, MR, and angiography, we examined 12 cases of spontaneous carotid cavernous fistula, five of whom presented only with cranial nerve palsies. RESULTS In t...
